We are committed to the principles of recovery and to empowering you to live life to its fullest. Safety is key to a fulfilling life. Upon admission, and whenever a resident has expressed intent to hurt themselves or others, by their words or their actions, we will provide additional support (what we call ‘safety supervision’).
The clinical team will review the continued need for safety assistance with you every 24 hours. During that time, we will work with you to help address safety concerns. If you disagree with your need for support, please discuss that with any member of your recovery team. The levels of safety supervision are as follows:
1:1 Suicide/Self-Harm
- Staff will keep you in sight and remain a minimum of two arms lengths away from you. You will be supervised in the bathroom and staff will accompany you into any closed area (offices, public restrooms, etc.).
- If you have anything in your possession that may be used for self-harm staff will request that you forfeit those items. If staff requests are not successful, staff will physically intervene.
- Staff will conduct a room search with you when you are placed on this supervision status to be sure you have no other dangerous objects. Staff may conduct additional room searches with you if the need arises.
- You will not be able to participate in community outings while on this status.
1:1 at the Residence
- Staff will keep you in sight and remain within 10 feet of you. While in your bathroom, the door must remain ajar.
- Your bedroom door will be kept open and a staff person will be sitting just outside.
- You will not be able to participate in community outings while on this status.
- Staff will maintain visual contact with you.
- You may participate in community integration outings with permission from clinical and/or nursing staff.
15 Minute Checks
- Staff will check in with you every 15 minutes. We will ask that you keep your bedroom door cracked so that we can check in on you without disturbing you.
- You may use the bathroom unsupervised, but staff will knock on the door and expect a response when a check in is due.
- We ask that you remain on-site while on this safety status, except to attend treatment related appointments. Staff will help you decide if an outing you wish to have while on this safety status is treatment related. You mat then participate in that outing with permission from clinical or nursing staff.
1:1 In the Community
- You may attend community outings with nursing and/or clinical approval.
- A staff person will remain within 10 feet of you on your outing.
With Staff in the Community
- You may attend community outings with a group of residents.
- A staff person will accompany the group during the outing.
